114 Spatial transcriptomic profiling of autoimmune bullous diseases reveals disease-specific pathogenesis

Pemphigus Vulgaris (PV) and bullous pemphigoid (BP) are autoimmune diseases where autoantibodies target structural proteins of desmosomes or hemidesmosomes, respectively. While PV BP both mediated by autoantibodies, direct comparisons the individual mechanisms inflammatory cell infiltration loss epithelial adherence have not been explored in depth. By applying a cutting-edge spatial transcriptomic platform, we profiled signatures epidermis dermal infiltrates from 4 5 patients. In PV-infiltrated immune cells, genes related to antigen processing presentation major histocompatibility complex (MHC) class I were upregulated (p-value<0.01) compared BP, implying importance autoantigen PV. macrophage mast activation pathways activated addition neutrophil migration, suggesting involvement diverse innate components disease pathogenesis. At epidermal level, blistered regions showed upregulation motility, locomotion, apoptosis, along with downregulation proliferation. However, only few common between blisters, different dissociation response autoantibody deposition. Overall, profiles provide unique insights into shared distinct pathomechanisms that may inform on future therapeutic targets.
